Q: What is the best option for tightening the areas around the jaw line? Some people have told me that I am too young for a facelift. But Thermage did not work for me, suggestions?
A: The best way to tighten skin on the jaw line and the lower neck can be different for each person. It depends on each person’s own condition. For someone who has a lot of excess fat and lax skin in the neck, but has a strong jaw line, liposuctioning in the office followed by a laser might be the best option for treating just that portion of the neck. There are a few newer devices such as Pelleve made by Ellman, SmartLipo, and Ulthera that you may be interested in researching.
We are currently working on a new product which uses a laser under the skin to tighten the skin following liposuction. It has showed great results in our current trial and may, for some, lessen the need for surgical procedures. However, for those who are looking for an even less invasive approach to the jaw line, the best option may be to strengthen the jaw line by placing filler in the jaw line. Doing this actually gives the impression of a more swan-like and defined jaw line. This in itself may be enough to achieve the results you are looking for. There are some topical radio frequency devices which could be helpful. We are currently involved in a trial of a device, and so far it seems to be working quite well in most patients in improving the tightness of the skin. However, it is a modest improvement. Another study that we are conducting utilizes an ultrasound topical device. The procedure takes 1-hour and is done in the office. After one or two treatments, the majority of the patients are seeing improvement and a tightening of the jaw line. To what extent this can be used to tighten is still to be determined. However, we should have stronger data to support the best options soon.
